Caring Connections–an ALF peer to peer support program connects patients and caregivers facing the challenges brought on by living with liver disease with others who have been facing these same challenges. Using our new online peer mentoring platform, you are matched, one-to-one, with a trained peer mentor and can communicate safely and securely by messages or video chat. This online platform makes it easy to share your questions, experiences, and challenges with a trusted peer who understands what it's like to be in your shoes, no matter where you are in your liver disease journey. Peer mentors can share their experiences with the challenges of a new diagnosis, treatments, transplantation, family and social relationship challenges, and more.
Caring Connections—an ALF peer to peer support program, is open to any adult with liver disease or with a liver transplant or a caregiver for someone with liver disease. All participants must be located in the United States and be over the age of 18.
Mentors provide their mentees with at least one hour of support and interaction per month or as much as mutually agreed upon. Most of this interaction will take place via phone, text, email, and other web-related tools (for example, Zoom). Mentors will work with their mentees to determine what kind of support will be most useful—specific feedback related to their condition, how they are handling any stress, family, or social circle issues. This is always done with the understanding that the mentor is not a medical or counselling professional, but someone sharing their own personal experiences.
Mentees will first complete a profile with information about their condition(s). Age, ethnicity, and the other topics can be useful but are not required. ALF will use this information to match you with a suitable mentor for your needs! Mentees play an active role in setting up meetings and participating in the peer to peer relationship. Mentees can expect open and honest dialogue with their mentor about any challenges or concerns.

No, this program is free.
No, you do not have to have a computer or a smartphone. However, if you use a computer, we’ve worked hard to make this experience as simple as possible. There are many guides and videos to help move you through the process. There is also an administrator you can reach out to with questions.
No, if you feel you are not getting the full benefit of your mentor connection, we can help you find a different mentor or you can opt out completely. However, we hope you will give the program at least six months of participation. There will be monthly check-ins with the administrator to see how things are going.
Yes, you can include how much or as little information about yourself as you feel comfortable. This program is confidential, and we never share anyone’s information outside of it. All of our mentors have been vetted and trained and understand the importance of the comfort of this program.
